COPD Melbourne: Diagnosis, Treatment and Specialist Care | RSDC

Chronic Obstructive Pulmonary Disease (COPD) is one of Australia’s most common and serious lung conditions, affecting approximately one in 13 Australians over the age of 30. If you or someone you love has been diagnosed with COPD in Melbourne, early specialist care is essential to slow progression, reduce flare-ups, and maintain quality of life.

At RSDC, our respiratory physicians provide comprehensive COPD assessment and management across five Melbourne locations.

What Is COPD?

COPD is a chronic inflammatory lung disease that causes obstructed airflow from the lungs. It is an umbrella term that includes two main conditions:

  • Chronic bronchitis — long-term inflammation of the bronchial tubes, causing persistent cough and mucus production
  • Emphysema — gradual destruction of the air sacs (alveoli), reducing the lungs’ ability to transfer oxygen into the bloodstream

Most people with COPD have features of both. The disease is progressive — meaning it worsens over time — but with the right treatment and management, progression can be significantly slowed.

COPD Symptoms

COPD symptoms often develop slowly and are frequently dismissed as a normal part of ageing or being unfit. Key warning signs include:

  • Persistent cough, often with mucus (particularly in the morning)
  • Shortness of breath, especially during physical activity
  • Wheezing or a whistling sound when breathing
  • Chest tightness
  • Frequent respiratory infections
  • Fatigue and reduced exercise tolerance
  • Unintended weight loss in later stages

If you experience any of these symptoms — particularly if you have a history of smoking — it is important to seek assessment from a respiratory specialist.

What Causes COPD?

The primary cause of COPD is long-term exposure to lung irritants. In Australia, the most common cause is cigarette smoking, which accounts for approximately 75% of cases. Other causes include:

  • Long-term exposure to occupational dusts, chemicals, and fumes (miners, construction workers, farmers)
  • Air pollution — both indoor (biomass fuel burning) and outdoor
  • Genetic factors — a rare condition called Alpha-1 Antitrypsin Deficiency can cause COPD even in non-smokers
  • Poorly managed or severe asthma in childhood

How Is COPD Diagnosed?

COPD is diagnosed through lung function testing. The primary test is spirometry — a quick, non-invasive test that measures how much air you can breathe in and out, and how fast. At RSDC, spirometry and comprehensive respiratory function testing (RFT) are available at all locations.

Your RSDC respiratory physician may also order:

  • Full lung volume measurements
  • Diffusing capacity (DLCO/TLCO) testing — to assess gas transfer efficiency
  • Chest X-ray or CT scan
  • Arterial blood gas analysis
  • Six-minute walk test to assess functional capacity

Severity is graded using the GOLD staging system (Grade 1 to 4), which guides treatment decisions.

COPD Treatment Options

While there is currently no cure for COPD, effective treatment can significantly reduce symptoms, prevent exacerbations (flare-ups), and improve daily function. Treatment is tailored to your severity and individual needs:

Smoking cessation — the single most effective intervention for slowing COPD progression. RSDC offers smoking cessation support and referral.

Bronchodilator inhalers — the cornerstone of COPD pharmacotherapy. Short-acting bronchodilators (SABAs, SAMAs) provide immediate relief; long-acting bronchodilators (LABAs, LAMAs) are used for ongoing control. Combination inhalers simplify treatment.

Inhaled corticosteroids (ICS) — added for patients with frequent exacerbations or features of asthma-COPD overlap.

Pulmonary rehabilitation — a structured exercise and education programme that improves breathlessness, exercise tolerance, and quality of life. Strong evidence supports pulmonary rehab in all COPD patients with breathlessness.

Oxygen therapy — for patients with severely low blood oxygen levels (hypoxaemia), long-term supplemental oxygen reduces mortality.

Vaccinations — annual influenza vaccine and pneumococcal vaccine are strongly recommended for all COPD patients to prevent respiratory infections that can trigger exacerbations.

Exacerbation management — COPD flare-ups (acute exacerbations) require prompt treatment with bronchodilators, corticosteroids, and sometimes antibiotics. Early treatment reduces hospitalisation risk.
